Introducing Fetal Blood Type Screening in Ontario

Prompted by Ontario Health’s Health Technology Assessment (HTA) on non-invasive fetal RhD genotyping, we are pleased to share that fetal blood group genotyping will be available in Ontario starting in January 2026.

This new opportunity provides screening for fetal RhD status and other blood group antigens to assess for fetal blood group incompatibility in pregnancy.  This screening will allow for the continued prevention of hemolytic disease of the fetus and newborn (HDFN) while reducing unnecessary health-care utilization by enabling:

  • a more effective and efficient approach to RhD Immune Globulin (RhIG) prophylaxis for RhD-negative pregnancies, and
  • appropriately targeted follow-up care for higher-risk alloimmunized pregnant individuals.

What tests will be available and who is eligible for testing?

Two non-invasive, cell-free DNA tests will be publicly-funded for pregnant Ontario residents.

  • Fetal RHD screening for RhD negative pregnant individuals.
  • Alloimmunized fetal blood group genotyping for pregnant individuals with antibodies D, C, c, E, and/or K (Kell).

What are the key benefits of this testing?

Tailored Care

Fetal RHD screening will predict the fetal RhD blood type and guide the need for RhIG, minimizing unnecessary use of this human blood product and promoting it's conservation.

Alloimmunized fetal blood group genotyping will detect fetal blood group antigens RhD, C, c, D and/or K (Kell) to identify pregnancies at risk of HDFN requiring further monitoring while allowing those carrying antigen negative fetuses to transition to routine care.

When will this testing be clinically available?

Testing is anticipated to be available in January 2026.
